Website Privacy Notice (Draft)

Draft for practice review. This notice describes the website and its appointment-request form. The source repository is a static website connected to a Vercel project. A Vercel contact function is configured in source to send submissions through Gmail, while a separate Cloudflare Pages handler belongs to a test project. The current production recipient, provider settings, tracking behavior, and retention period still need confirmation before publication.
Information submitted through the contact form
The website contact form asks for your name and one contact method, an email address or phone number, for an appointment or callback request. It does not ask for detailed health information or permission to send text messages. The Vercel handler in the source sends the request by email through Gmail to the practice address configured for that deployment. A separate Resend-based Cloudflare Pages handler is present for a test project and is not the production path identified in the current project records. As checked on October 26, 2026, the production setting sends each request to the practice management mailbox, through the practice's Gmail sending account.

Services and embedded content
The website does not use advertising or analytics tracking: it has no Google Tag Manager, Google Analytics, WordPress.com site statistics or Cloudflare Web Analytics. Its pages still load a few outside services to display content: Google Fonts; the Trustindex Google-reviews widget; the UserWay accessibility widget; and Google Maps links. The contact page also loads Google reCAPTCHA to help block automated submissions. These services may receive your IP address, browser and device details, and the pages you view, and some may set cookies or similar identifiers in your browser. We have not confirmed which of them apply to a given visit.
Cookies, Do Not Track, and third-party tracking
The website does not currently show a cookie banner, because it does not run analytics or advertising tags. The outside services listed above load when a page opens, so you can limit them with your browser settings, a content blocker, or the Global Privacy Control signal in browsers that support it; the website does not currently change its behavior based on that signal or on browser Do Not Track signals. Google, Trustindex, UserWay and the other providers named above handle information under their own privacy terms.
Use, storage, and requests
The form is intended for appointment and callback requests. The source code shows the submitted name and chosen contact method are emailed to the practice. The current production recipient, staff access, retention period, and deletion process have not been verified. Notice of Privacy Practices
This website notice is separate from the practice’s Notice of Privacy Practices for patient health information. It does not replace that notice. The patient-information page currently points to an older educational template dated April 14, 2003. The practice needs a current, practice-specific Notice of Privacy Practices before that patient notice can be treated as current.
